TO RESERVE A SFSD FACILITY FOLLOW THESE STEPS.

1. See if the facility is available by checking the Web Calendar.

• You may also want to check this calendar for conflicts with other major events.

2. Read and agree to adhere to all Rules and Regulations of facility use.

3. Complete the proper form (digital or paper format is your choice):

• Digital Forms ... no need to mail!

  • Facility Request Form - ATHLETIC facilities (updated: 05-08-2013)

  • Facility Request Form - NON-Athletic facilities (updated: 05-08-2013)

  • School Staff Facility Request Form (updated: 06-06-2013)

• Paper requests can be completed and submitted to the address below.

  • Facility Request Form - ATHLETIC facilities

  • Facility Request Form - NON-Athletic facilities

4. Your request will be reviewed by the administration.

5. An estimate of the charges will be forwarded to you.

6. Approved requests will be posted on the Web Calendar.

PLEASE NOTE ALL REQUESTS MUST BE SUBMITTED TWO WEEKS IN ADVANCE OF REQUESTED DATE.
